Shower head pipe from wall

I'd like to change my shower head pipe to make it higher but when I took the old one a apart it's a threaded flange with a black disk behind it which tightens it to the wall. Is anyone aware of a way to change the shower head arm with out accessing the wall from behind. I also will be converting a three handle supply to a single handle which I know I need access from behind.
Like Steve said, unless you can do surgery through a 1" hole you will have to open things up.
I would wait until you have the new faucet and do everything at once. I would recommend getting a length of copper pipe and a shower elbow they screws into a header fastened between 2 studs. That way everything is secure.
